The National Disability Insurance Scheme (NDIS), established by the Australian government, aims to ensure people with disabilities have equal opportunities to thrive in life.

At NDSC, our mission is to support the objectives of the NDIS by offering specialised care tailored to the needs of participants with physical health conditions. Our team of skilled and compassionate support staff prioritises the needs and aspirations of participants, fostering equal opportunities while encouraging personal growth and independence in a familiar, supportive environment.

Our Goals & Objectives

  • Provide Continuity of Care– Care begins with direct management involvement with the participant, followed by hand-off to dedicated carers.  Continuity is provided by matching carers with participants on an ongoing basis.  When carers are unavailable, management covers, thus maintaining continuity.

 

  • Optimum setting for Care– Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Short Term Accommodation (STA), Medium Term Accommodation, hospital or in the home. 

 

  • Participant Satisfaction– Achieve and maintain high levels of participant satisfaction by regularly assessing and exceeding expectations, soliciting feedback, and implementing improvements based on participant input.

 

  • Quality Assurance– As a Registered NDIS Care Provider, NDSC maintains the highest standards of quality and safety in all aspects of our operations, including caregiver training, service delivery, and compliance with regulations and industry best practices.

NDSC Experience

Discharge of long stay patients to Community Care – What we learnt working with PA Hospital Brain Injury Rehabilitation Unit (BIRU)

Working alongside PA Hospital’s Brain Injury Rehabilitation Unit has taught NDSC that the best outcomes come from close collaboration with hospital teams, allied health professionals, and families.

Our approach starts with concurrent care and training in hospital, involving all stakeholders in discharge planning to ensure continuity and reduce stress.

Every support worker is introduced to the participant before discharge and receives person – specific training. Our clinical management team works the initial shifts, then trains the wider team alongside the person in our care.

We partner with allied health to implement exercise regimes and therapeutic measures — our support workers’ clinical backgrounds make this seamless. This shared sense of purpose keeps everyone aligned: participants, families, hospital teams, and allied health working toward the same goals.

This approach has been shown to reduce re-admissions to hospital. In the case of NDSC, no NDIS participants in our care have been subject of social admissions or any other re-admission.
